Health check schedule and questionnaires

A list of available questionnaires about a child's health and development. Health check questionnaires are currently only available for residents of New South Wales and Tasmania.

Please note: currently all health check questionnaires are read-only, meaning no one is able to make any changes, add or create new questionnaires at this time.

 

How to find the health check questionnaires in your record

Get started: 

  1. Sign in to My Health Record:


     
  2. Select the record you want to access.
  3. Select Documents from the top menu.
  4. Select My Childhood Development and select Health Check Assessment Schedule.
  5. Choose your state from the drop-down list and click on Select.
  6. A list of read-only and completed questionnaires will be displayed.
