Advance care document custodian

Someone who holds a copy of your advance care planning document.

Making sure your wishes are known

An advance care document custodian is either a person or an organisation that holds a copy of your advance care planning document. A healthcare provider may contact your custodian to get access to your documents.

People who can access your record will be able to view your advance care document custodian information. Find out more about who can access a record.

​​​​​Details of up to two custodians can be stored in your record. Saved custodians will be displayed on the Advance Care Document Custodian page

Advance care document custodian consent

The Advance Care Document Custodian section in your Advance Care Plan is where you can add the details of the contact person, or organisation. You must obtain their consent before uploading their details to your record.

Storing this information will help healthcare providers access your custodian’s details if they are needed.

How to add an advance care custodian

Getting started: 

  1. Sign in to My Health Record:


     
  2. Select Documents in the menu and navigate to Advance Care Planning
  3. Select the Add a Custodian button.
  4. Select whether the custodian is a person or an organisation.
  5. Enter the custodian's address details and their name.
  6. Select the contact method preferred by the custodian from the dropdown menu. 
  7. Enter either an email or phone number for the custodian and whether the email or phone number provided is for personal or business use
  8. Repeat the above steps to add a second advance care custodian
  9. Select Save

How to edit advance care planning custodian details

Getting started: 

Follow these steps if you need to change your custodian information:

  1. From Documents in the top menu, select Advance Care Planning
  2. Select Add a Custodian
  3. In the Advance Care Document Custodian section select edit
  4. Update the information as required
  5. Select Save

If you want to remove a custodian, select Remove next to their name on the Advance Care Document Custodian page.
