Care and support plans

What are care and support plans?

Care and support plans are comprehensive documents describing agreed goals of support, and outlining planned medical, nursing and allied health activities. 

These documents can be viewed within My Health Record and via the my health app. 

How to view care and support plans

The following steps outline how to view the different care and support plans viewable in your record:

  1. Once you have signed in to your record, select the Documents tab
  2. Select Care and Support Plans
  3. Scroll down to Care and Support Plans section where you can select the document you wish to view.
  4. Select the document you wish to view

Types of care and support plans

My Aged Care support plans

A My Aged Care support plan records what you discussed and agreed during your My Aged Care assessment such as: your strengths, difficulties, goals, what you would like to achieve, and what preferences you have for your care services.  

The support plan shows what aged care services you are approved for at a point in time based on your needs and abilities – not what aged care services you are receiving.
