Important health information that you and your healthcare provider organisations have added, all in one place.

Sign in to My Health Record via myGov

You will need to have My Health Record linked to myGov to access it.

Having your health information with you throughout your life helps ensure you receive the most accurate healthcare possible.

What can be uploaded and viewed?

Documents uploaded by your healthcare providers

Healthcare providers like doctors, pathologists, and pharmacists can upload clinical documents to your record including:

Better and easier access to your information

Many providers are already uploading documents to your record. Changes are being proposed to require pathology and diagnostic imaging reports to be uploaded by default, unless you tell the provider not to or a valid exception applies. It will give you and your healthcare providers better access to this information. Learn more.

Information from Medicare

Your record can include certain Medicare information held by Services Australia, depending on the settings that you choose.

Medicare information can include:

  • Medicare Benefits Schedule or Department of Veterans' Affairs claims
  • Pharmaceutical Benefits Scheme or Repatriation Pharmaceutical Benefits Scheme claims
  • Australian Immunisation Register
  • Australian Organ Donor Register information.

Incorrect or missing information

It’s important that you check that all your health information is up to date and correct. Learn more about what to do if you have incorrect or missing information in your record.

Add information to your record

You can add the health information that you think is important or may want to have available in an emergency, including:

  • contact numbers and emergency contact details
  • current medicines
  • allergy information (including previous reactions)
  • personal health notes
  • Indigenous status
  • Veterans’ or Australian Defence Force status
  • your advance care plan or contact details of your custodian
  • preferred language
  • country of birth.
